Description

The intent of this Request for Proposal (RFP) is to award one (1) contract for Consultative services of a Optometrist for the Non-Insured Health Benefits (NIHB) Program. The services will be on an as and when required basis. The initial contract period is three (3) years from contract award, with up to two (3) additional one (1) year option periods.
