The proponent should submit in a second (file) envelope, one (1) original of the completed Appendix C – Rate Bid Form(s) along with any additional price breakdown information that you wish to provide.
The two envelopes (files) should be clearly marked as to their contents and addressed in accordance with Section 1.5.1.
