Description

This Request for Proposals (“RFP RM7-05”) is an invitation by Brock University (the
“University”) to prospective proponents to submit proposals for the provision of Employee
Family Assistance Program. The term of the agreement is to be for a period of (3) Three years with an option in favour of the University to extend the agreement on the same terms and conditions for an additional term of up to (3) Three x one year optional extension periods. It is anticipated that the
agreement will be executed on or around May 1, 2018.

Administrative and Technical Submission- The proponent should submit the following information in Envelope #1:
i. Appendix B - Submission Form
ii. Appendix D - Reference Form
iii. Bid Submission Requirements as requested in Appendix E – RFP Particulars.

Proponents shall not include any cost information in the body of the administrative and technical proposal or it will be rejected. Do not include the Appendix C – Rate Bid Form(s), or any price information in this portion of your proposal
