Notice is hereby given that Fraser Health Authority (the “Health Authority”), through their agent
Provincial Health Services Authority (“PHSA”), intends to directly award a Contract to the following
supplier to provide 2 units of LifePulse High Frequency Jet Ventilator with Whisper Jet Patient Box for
Surrey Memorial Hospital NICU.
