195128 - Staffed Residential and Community Inclusion Supports - Trail BC

Notice is hereby given by Community Living British Columbia (CLBC), of its intent to enter into a contract with ECLIPSE FOUNDATION FOR INDIVIDUALIZED SUPPORTS (“Eclipse”) to provide the following SERVICE TYPES for its TRAIL PROGRAMS:

1. STAFFED RESIDENTIAL – two staffed living homes CONSOLIDATING TO ONE LOCATION OF SERVICE. The home has 6.35 FTE based on a 37.5 hour week and 238 gross fixed service levels per week. The new home has the capacity for four (4) funded spaces, 3 of which are moving from other resources and the 4th individual will utilize a newly created vacancy with no additional funding required in the new resource.

1. HOME-BASED COMMUNITY INCLUSION – has 1.12 FTE based on a 37.5 hour week with a total of 42 gross fixed service level hours per week. This program will support all four (4) individuals in the home.

The total value of the contract to supply the above SERVICE TYPES is expected not to exceed a total contract value of $1,656,000.00 ($69,000 per month) for the initial 2 (two) year term of the contract subject to potential global uplifts which may increase the total contract envelope during the initial two-year term.
