Description

The objective of this Request For Information (“RFI”) is to:
• examine new models of care for individuals who have complex service needs including those at risk of hospitalization, and/or transitioning from acute care; and,
• assess how soon such models can be implemented to support these individuals.

This RFI is not a competitive bid process and will not result in a contract award.
