Description

This Negotiated Request for Proposals (“NRFP”) invites Proposals from prospective Proponents for the provision a community-based service provider (or providers) to provide up to six (6) units of transitional/respite housing for clients who otherwise regularly reside within Island Health provided complex care housing (“CCH”) sites, as further described in this NRFP. The program size may change in the future depending on the available annual funding resources.

In addition to temporary furnished housing units, proponents will be expected to provide other supports to clients accessing these units, and supports may include: monitoring/supervising daily activities, providing emotional supports, implementing pre-established care plans), and providing staff 24/7. Clients accessing these units may stay for as little as twenty four (24) hours up to ninety (90) days, with flexibility if a client’s care plan would be best supported beyond the ninety (90) days.
