Description

• The purpose of this procurement is for the Ministry of Health to establish a new contract for Plastic Sleeves for Ontario’s Publicly Funded Vaccine Programs.

• The clear Plastic Sleeve is provided to Ontarians along with an immunization card, which allows them to keep track of their personal immunization records or those of their children / dependents.
Scope of Work
Plastic Sleeves Technical Specifications:

• Plastic sleeves for immunization cards must be clear vinyl or clear polypropylene.
• The inner dimension of the sleeve must be: 6 inches height by 4.25 inches width.
• The sleeve must be sealed on three sides and have an opening on one of the width sides to allow insertion of Ontario’s immunization card.
• The plastic sleeve must allow insertion of Ontario’s immunization card with dimensions: 5.5 inches height by 3.75 inches width.
• The plastic sleeves must be packaged in bundles of 20, 50 or 100.
• The Ministry of Health requires 355,000 Plastic Sleeves for the first firm-year period of the contract; Additional quantities may be required and may be specified by the Ministry of Health from time to time.
