Nature of Requirements:
The following is a summary of the statement of work for this requirement.

Correctional Service Canada (CSC) Health Services require services from primary care physicians for the following institution(s) in Ontario Region:

Bath Institution
Joyceville Institution
Millhaven Institution
Regional Treatment Centre

Objectives: The Contractor (Primary Care Physician (PCP)) must provide integrated patient centred medical services to patients (inmates) in collaboration with the Institution's inter-disciplinary health care team.

Deliverables: The PCP must provide integrated, trauma informed, culturally appropriate Primary Care health services to patients as described in the statement of work.

Any conditions for participation of suppliers not specified in solicitation documentation: None.

Estimated quantity of commodity: See solicitation document statement of work and basis of payment.

Duration of Contract and Time Frame for Delivery:
Period of the Contract: The Work is to be performed during the period of 2024-09-01 to 2027-08-27.
