Physical Bid Submission: The proponent should submit one (1) original and two (2) copies of the following information in Envelope #1. The original copy should be labelled as such:
                              i.        Appendix B - Submission Form
                             ii.        Appendix D - Reference Form
                            iii.        Bid Submission Requirements as requested in Appendix E – RFP Particulars.
                           iv.        One (1) Electronic Copy of your Bid Submission.
Proponents shall not include any cost information in the body of the administrative and technical proposal or it will be rejected. Do not include the Appendix C – Rate Bid Form(s), or any price information in the electronic version of your proposal.
