Invitation to Proponents

This Request for Supplier Qualification ("RFSQ") is an invitation to suppliers (the “Proponents”) to submit proposals (the “Proposals”) for Virtual Patient History and Assessment Screening Tool (“VPHAST”) described in Section 2 Description of Requirements. This RFSQ is issued by the Ontario Telemedicine Network (herein referred to as “OTN”), a not-for-profit Broader Public Sector (“BPS”) corporation primarily funded by the Province of Ontario through the Ministry of Health (“MOH”), on behalf of itself and the following participants (the “Participants”):

This RFSQ will result in the qualification of all Proponents who achieve the required minimum scores as described in this RFSQ for a VOR arrangement for VPHAST. This RFSQ will result in the Proponents being qualified on a VOR arrangement with the intent of these Proponents initially being invited to enter into a Master Agreement under the VOR arrangement. For clarity, any Proponent who does not participate in the prequalification or does not appear on the list of Proponents qualified on a VOR arrangement will be excluded from opportunities.

Notice to all Proponents: OTN strongly urge you to read the document in its entirety so a full understanding of each stage can be absorbed and assessed. There are several key dates and requirements outlined within the RFSQ.
