Invitation to Proponents
This Request for Supplier Qualification ("RFSQ") is an invitation to suppliers (the “Proponents”) to submit proposals (the “Proposals”) for Mental Health and Addictions Solutions described in Section 2 Description of Requirements. This RFSQ is issued by the Ontario Telemedicine Network (herein referred to as “OTN” or “Issuer”), a not-for-profit Broader Public Sector (“BPS”) corporation primarily funded by the Province of Ontario through the Ministry of Health and Long Term Care (“MOHLTC”), on behalf of itself and the following participants (the “Participants”):

- Waterloo Wellington LHIN
- Hamilton Niagara Haldimand Brant LHIN
- Central West LHIN
- Mississauga Halton LHIN
- Toronto Central LHIN
- Central LHIN
- Central East LHIN
- South East LHIN
- Champlain LHIN
- North Simcoe Muskoka LHIN
- North East LHIN
- North West LHIN
- The health and community care providers within the above LHINs
- Canada Health Infoway
This RFSQ is an opportunity for Proponents to submit proposals for Mental Health and Addictions Solutions in order to qualify for a Vendor of Record (VOR) for these and future categories that may be added. This RFSQ contains general requirements and specific requirements for Category 1: Mood and Anxiety and Category 2: Addictions. Proponents may apply to one or both of the categories.
OTN anticipates adding up to eight (8) additional categories over the term of the Agreement.
This RFSQ will result in the qualification of all Proponents who achieve the required minimum scores as described in this RFSQ for a VOR arrangement for Category 1: Mood and Anxiety and/or Category 2: Addictions. This RFSQ will result in the Proponents being qualified on a VOR arrangement with the intent of these Proponents initially being invited to enter into a Master Agreement under the VOR arrangement. For clarity, any Proponent who does not participate in the prequalification or does not appear on the list of Proponents qualified on a VOR arrangement may be excluded from opportunities.
The Scale and Spread Initiative:
A decision will be made following the evaluation of Category 1 Solutions to select a solution for the immediate initiative “the Scale and Spread Initiative” as described in Schedule G - Scale and Spread Initiative for Mood and Anxiety. Based on assessment of whether the Solution and the successful Proponent have met the objectives set out in this RFSQ, at the end of the Scale and Spread Initiative, a decision will be made to either proceed with the same Proponent(s) to the next phase and ultimately full Solution, or, among other options, to end the engagement and go to a second stage procurement under the RFSQ.
