Description

OTN is seeking to enter in a contractual arrangement for the provision of Design-Thinking Services; beginning with an initial scope of work for:

“the co-design of a comprehensive digital chronic disease self-care model that both patients and primary care providers will want to engage in and thereby support rapid adoption throughout the province.”

Using co-design approaches, the services will be based on a design thinking methodology including, for example, such services as discovery workshops, healthcare process mapping, and environmental scans; and.
Stakeholders for the co design should include but is not limited to a range of PC providers, patients and caregivers (and personas), associated home and community partners, and OTN.

For clarity, OTN’s first Design Thinking SOW is for the Co-Design of a Primary Care Digital Chronic Disease Self- Management (DCDS) Model. Other Design Thinking SOWs may be executed during the term of the Agreement.
