Invitation to Proponents
This Request for Supplier Qualification ("RFSQ") is an invitation to suppliers (the “Proponents”) to submit proposals (the “Proposals”) for Chronic Pain Virtual Care Solution described in Section 2 Description of Requirements. This RFSQ is issued by the Ontario Telemedicine Network (herein referred to as “OTN”), a not-for-profit Broader Public Sector (“BPS”) corporation primarily funded by the Province of Ontario through the Ministry of Health (“MOH”), on behalf of itself and the following participants (the “Participants”):

This RFSQ will result in the qualification of all Proponents who achieve the required minimum scores as described in this RFSQ for a VOR arrangement for Chronic Pain Virtual Care Solution. This RFSQ will result in the Proponents being qualified on a VOR arrangement with the intent of these Proponents initially being invited to enter into a Master Agreement under the VOR arrangement. For clarity, any Proponent who does not participate in the prequalification or does not appear on the list of Proponents qualified on a VOR arrangement will be excluded from opportunities.

Notice to all Proponents: OTN strongly urge you to read the document in its entirety so a full understanding of each stage can be absorbed and assessed. There are several key dates and requirements outlined within the RFSQ.
