RFP-SS-002-SH - Remote Home Monitoring

Manitoba’s Clinical & Preventive Services Plan (CPSP) identified the delivery of remote home monitoring services as a critical priority to better support the needs of Manitobans who can be supported by care at their home. This service is expected to reduce use of acute care resources such as emergency department visits by providing patients with chronic disease such as CHF and COPD with monitoring and health coaching to support them in managing their condition. In addition to these, several of the provincial clinical teams identified conditions that may benefit from this type of delivery model such as: intensive virtual health counselling, diabetes, post-stroke and mental health conditions.

The demands of the COVID-19 pandemic have accelerated the urgency for the remote home care monitoring solution to be implemented.  This platform can enable services to monitor COVID-19 patients at home outside of an acute setting reducing the demand for hospital based services.

This Request for Proposals (the “RFP”) is to solicit proposals from qualified vendors to offer a provincial remote home monitoring solution for Manitobans being managed at home with low to mid acuity COVID-19 patients in order to reduce the impacts of COVID-19 on hospital capacity while still actively monitoring their health.  This initial implementation will be  a foundation and extended to support  chronic diseases and other conditions as indicated above.
