Description

The Orléans Health Hub is a unique and innovative concept that integrates, under one roof, a range of specialized and community care focused on the individual and their loved ones.
The Hub is linked to the development of a new model of care, which will promote a philosophy of care and services called integrated and person-centred. Integration will be done from the users' point of view and according to their needs and preferences. It will be based on close collaboration between the people cared for and the professionals who will support them.
This new model of care means that the design team must “invent” the new model as the project takes shape.

Each of the OHH clinical partners have unique processes and technologies to support delivery of health care services. As well, there are many provincial applications which have been deployed or are planned. The proposed technology solution must take these entities into consideration to ensure alignment, ease-of-use and cost-effectiveness with the ultimate aim of providing integrated care to all patients of the OHH.
