The Proposal shall include:
1. Company Profile: Please provide a brief outline of your company, including a list of services, and where you feel your company excels. Please include:
• Full legal business name and address
• Number of full-time employees
• Date company was founded and company history
• Areas of specific expertise and any experience working with agricultural, supply managed or dairy industry clients.
2. Resources and Equipment:
• A detailed description of the trucks, body and reefer.
• A detailed description of how temperature will be monitored/controlled by driver on route.
• Staffing and back up equipment plans.
3. Pricing: A detailed and separate pricing proposal for all three options in APPENDIX A, as well as incidental fees and triggers for those fees. Flat rate per route or a $/km rate per route and applicable fuel surcharge program.
4. Proposed Routes: Detailed description of proposed routes if different than current practice. DFO encourages innovative solutions be brought forward. Appendix D shows current route information.
5. Routes must be based on specific time windows for earliest pick-up times at each depot to ensure all milk transporters can place samples in fridge prior to pick-up by the Sample Transporter. (See Appendix E)
6. Identify any resource limitations: Confirm whether there is anything that could adversely affect your ability to support the/these contracts or perform the work over the next three (3) years and whether subcontracting is contemplated/required (and the details of which subcontractors the proponent intends to use, name, experience, location, rate, context).
7. Differentiators: Please provide a summary outlining what makes your company different or ideally positioned to support DFO’s Milk Sample Transportation Services.
8. Financial Information: Provide sufficient information to evidence your suitability to enter into a long-term contract with DFO. This information shall include any factors that either tend to guarantee financial stability or potentially detract from your ability to execute a long-term agreement.
9. Term: The Contract between DFO and the successful Proponent will be for an initial term of three (3) years. DFO may opt to extend the Contract for up to two consecutive one (1) year terms, provided it gives the successful Proponent written notice at least thirty (30) days before the then current term expires.
